Here at Global Health we are committed to making sure that managing and navigating your healthcare is made easier.

We go beyond insuring your ability to pay for the medical care you need. Case management focuses on assisting or supporting you and/or your caregivers by coordinating your care, helping you develop a care plan, and advocating for your needs.

You may benefit from case management if you have a complex condition or if you have multiple chronic medical and/or behavioral health conditions. Current programs include:

  • Safe opioid use
  • Prenatal/post-natal

Our case managers work on your behalf collaborating with each member of your care team to promote quality care with a cost-effective outcome. Your care team may include your PCP, specialists, pharmacist, home health providers, durable medical equipment supplier and others depending on your specific needs.

GlobalHealth case management also provides members with resource materials from recognized organizations such as American Heart Association, American Diabetes Association, and the CDC, with a goal of educating our members on their disease process, the importance of medication adherence, and treatment options. The goal is for the member to learn how to self-manage aspects of care, as the provider deems appropriate, and ultimately become independent.

Participating in case management is optional. It will not cost you anything extra, and you can opt out of the program at any time. Your health plan benefits will not change whether or not you choose to take part in this program.

To begin working with GlobalHealth Case Management, please submit a Case Management Self-Referral form or you can call us directly. You also can be referred by your caregiver, doctor, or other provider, such as a hospital case manager. We will contact you in 7-10 business days.
