Coordination of care is the process doctors use to ensure they know about other care you are getting. It involves creating a care plan, open communication of the plan, and delivering the care. It is important, so a patient gets the best and safest possible. Patients aren’t going through the same testing ordered by different doctors. Doctors aren’t prescribing medications that duplicate or interact with medications prescribed by another doctor.
Of course, all healthcare professionals must abide by privacy laws. The easiest way to make sure doctors are allowed to communicate with each other is to release your medical records. You will need to sign a Release form.
Coordination of care should be centered around you and your family. Care continuity includes medical and non-medical services, from acute to long-term care.
